Travel Vaccination FormIf you require any vaccinations relating to foreign travel, we have a set process for patients to follow.
This allows us to make certain that there is no confusion in the booking process and that all required information is obtained.Step 1
Please complete our Travel Vaccination Form form below.
Please complete one form for each person travelling.
Complete all sections and Submit the form to send it through to the surgery.
This will be automatically forwarded to our Nurse to check.Step 2
Once you have submitted the form, call our Reception and request a Travel Vaccination appointment with our nurse.
Our staff should ask you if you have completed and submitted a form online for the nurse.
If you haven’t they will direct you to this page to do so before booking.
The appointment with the nurse should be at least 6 weeks before you travel.
This allows time for the vaccines to work before travelling and also allows time for any “special” vaccines that require multiple injections.
If booking more than 6 weeks ahead is not possible, book an appointment anyway (for each person travelling) as soon as possible, the nurse will still check the form and may still be able to accommodate vaccines and advice, but be aware that we may need to cancel the appointment and direct you elsewhere.
